Job summary

OPCO Skilled Management is seeking an MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator in ensuring documentation complies with Federal, State, and Certification guidelines. The role coordinates the RAI process, ensuring timely MDS, CAAs, and care plans, and participates in surveys to address deficiencies.

The position requires RN or LVN licensure and nursing experience in a Skilled Nursing Facility, with strong knowledge of OBRA, Medicare/Medicaid, and the care

Qualifications

• RN or LVN license in the state of practice is required.
• Minimum 2 years of skilled nursing facility nursing experience preferred.
• Strong knowledge of case-mix, PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations and the MDS process.

Responsibilities

• Assists the Director of Nursing and RN Assessment Coordinator in ensuring documentation meets guidelines.
• Coordinates the RAI process for timeliness and completeness of MDS, CAAs, and Interdisciplinary Care Plan.
• Participates in regulatory surveys and corrects deficiencies.
• Reports audit trends to Quality Assurance Committee.
• Manages MDS submission tracking and state validation follow-up.
• Ensures accurate Assessment Reference Date and appropriate payment capture.
