Job summary

Talentify is seeking an MDS Coordinator to assist the Director of Nursing and RN Assessment Coordinator in ensuring documentation meets Federal, State, and Certification guidelines. The coordinator will manage the RAI process, MDS submissions, and the interdisciplinary care plan.

The ideal candidate has RN or LVN licensure, at least 2 years of SNF nursing experience, and strong knowledge of Medicare PPS, Medicaid reimbursement, OBRA, and MDS procedures.

Qualifications

• Graduate of an approved Registered Nurse / Licensed Vocational Nurse program and licensed in the state of practice required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-M mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process.
• Knowledge of OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.

Responsibilities

• Assists the center in assuring adherence to Federal and State regulations and certification.
• Actively participates in regulatory or certification surveys and correct deficiencies.
• Reports trends from completed audits to the Quality Assurance Committee.
• Assures completion of the RAI Process from the MDS through the interdisciplinary care plan.
• Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms.
• Follows up with staff to assure compliance to standards of documentation.
• Completes patient assessments, data collection, and interviews staff as necessary.
• Facilitates accurate determination of the Assessment Reference Date reflecting patient needs and resources.
• Ensures timely submission of the MDSs to the State with proper follow-up on validation errors and maintains records.
