Job summary

OPCO Skilled Management seeks an MDS Coordinator to support the Director of Nursing and the RN Assessment Coordinator. You will ensure documentation aligns with Federal, State, and Certification guidelines and coordinate the RAI process for timely and complete MDSs and care plans.

The role requires an active RN or LVN license and at least 2 years of nursing experience in a Skilled Nursing Facility, with strong knowledge of Medicare/Medicaid reimbursement and OBRA/MDS procedures.

Qualifications

• RN or LVN license required in state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Solid knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement; OBRA regulations and MDS User's Manual knowledge.

Responsibilities

• Assists the center in adhering to Federal and State regulations and certification.
• Participates in regulatory surveys and correction of deficiencies.
• Reports trends from audits to the Quality Assurance Committee.
• Ensures timely and complete MDS/RAI process through care planning.
• Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms.
• Follows up with staff to ensure compliance with documentation standards.
• Completes patient assessments and data collection; conducts staff interviews as needed.
• Determines the Assessment Reference Date reflecting patient care needs and resource use for Medicare/Medicaid.
• Ensures timely submission of MDSs to the State and maintains validation records.
