Job summary

OPCO Skilled Management seeks a MDS Coordinator to assist the Director of Nursing and RN Assessment Coordinator in ensuring compliant documentation per Federal, State, and Certification guidelines. You will coordinate the RAI process and oversee the timeliness and completeness of the MDS, CAAs, and interdisciplinary care plan.

The role requires RN or LVN licensure and at least 2 years in a Skilled Nursing Facility, with strong knowledge of OBRA, MDS, and Medicare/Medicaid processes.

Qualifications

• Graduate of an approved registered nurse (RN) or licensed vocational nurse (LVN) program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Extensive knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Understanding of OBRA regulations and the MDS process.

Responsibilities

• Assist the center in ensuring adherence to Federal and State regulations and certification.
• Actively participate in the regulatory or certification survey process and the correction of deficiencies.
• Report trends from completed audits to the Quality Assurance Committee.
• Ensure timely submission of the MDSs to the State with proper follow-up on validation errors and maintain records.
• Initiate and monitor RAI process tracking, discharge/reentry and Medicaid tracking forms in PointClickCare.
• Follow up with staff to assure compliance with documentation standards.
• Complete patient assessments and data collection as instructed in the MDS User's Manual.
