Job summary

OPCO Skilled Management is seeking an MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator. The role focuses on ensuring documentation aligns with Federal, State, and Certification guidelines and coordinating the RAI process, MDS, CAAs, and the care plan.

The position requires RN or LVN licensure with at least two years in a Skilled Nursing Facility and strong knowledge of Medicare PPS, Medicaid reimbursement, and OBRA requirements.

Qualifications

• RN or LVN license in the state of practice required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS process, and Medicaid reimbursement.
• Understanding of OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.

Responsibilities

• Assist in assuring adherence to Federal and State regulations and certification.
• Actively participate in the regulatory or certification survey process and the correction of deficiencies.
• Report trends from completed audits to the Quality Assurance Committee.
• Assure the completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care.
• Initiate and monitor RAI process tracking, discharge/reentry and Medicaid tracking forms through PointClickCare.
• Follow up with staff to assure compliance to standards of documentation.
• Complete patient assessments, data collection, and interviews staff as necessary to assure good standard of practice.
• Facilitate accurate determination of the Assessment Reference Date reflecting patient care needs and resources.
• Ensure timely submission of the MDSs to the State with validation checks and maintain validation records.
