Job summary

Outfield Healthcare Partners is seeking an MDS Coordinator to ensure timely and accurate completion of the RAI and care management processes from admission to discharge in line with policy, procedures, and regulatory guidelines. The role supports the clinical department under the Director of Nursing and VP of Clinical Reimbursement.

Responsibilities include coordinating interdisciplinary MDS assessments, care plans, and ensuring proper documentation and data entry across state and federal

Qualifications

• RN must be licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Excellent knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and the MDS process.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on completing or not completing MDS assessments based on clinical criteria from the RAI User’s Manual.
• Assist in coordination of daily stand-up meetings and setting assessment reference dates.
• Comply with federal and state regulations for MDS processes.
• Monitor MDS and care plan documentation in medical records to support coding.
• Maintain current MDS status per guidelines and ensure data entry into MDS systems.
