Job summary

Outfield-Healthcare-Partners is seeking an MDS Coordinator to assist the Director of Nursing and RN Assessment Coordinator. You will ensure documentation meets Federal, State, and Certification guidelines and coordinate the RAI process across MDS, CAAs, and the interdisciplinary care plan.

The role requires RN or LVN licensure, with at least 2 years of SNF nursing experience, and strong knowledge of OBRA, Medicare PPS, and Medicaid reimbursement to support accurate care planning and compliant

Qualifications

• Graduate of an approved Registered Nurse / Licensed Vocational Nurse program and licensed in the state of practice required.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Through understanding of the Quality Indicator process and Minimum Data Set guidance.

Responsibilities

• Assists the center in assuring adherence to Federal and State regulations and certification.
• Actively participates in regulatory or certification survey process and correction of deficiencies.
• Reports trends from completed audits to the Quality Assurance Committee.
• Assures completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care.
• Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms through the PointClickCare system.
• Follows up with staff to assure compliance with documentation standards.
• Completes patient assessments, data collection, and staff interviews to assure good practice per the MDS User’s Manual.
• Facilitates accurate determination of Assessment Reference Date reflecting patient care needs and resources for Medicare/Medicaid.
