Job summary

Outfield-Healthcare-Partners is seeking a focused MDS Coordinator to support the Director of Nursing and RN Assessment Coordinator. You will help ensure documentation aligns with Federal, State, and Certification guidelines and coordinate the RAI process from MDS to the interdisciplinary care plan.

The role emphasizes regulatory survey participation, timely submission of MDSs, and tracking the RAI process in the PointClickCare system. A nursing license and SNF experience are preferred.

Qualifications

• RN or LVN license in state of practice required.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of OBRA regulations, MDS processes, and care planning.

Responsibilities

• Assist Director of Nursing and RN Assessment Coordinator to ensure documentation meets Federal, State and Certification guidelines.
• Coordinate the RAI process for timeliness and completeness of MDS, CAAs, and Interdisciplinary Care Plan.
• Participate in regulatory surveys and help correct deficiencies.
