Job summary

OPCO Skilled Management is seeking a skilled MDS Coordinator to assist the Director of Nursing and RN Assessment Coordinator in ensuring documentation meets Federal, State and Certification guidelines. The role centers on coordinating the RAI process to ensure timely MDS, CAAs, and interdisciplinary care plan completion.

The ideal candidate will have RN or LVN licensure in Texas, with at least two years in a Skilled Nursing Facility, deep knowledge of Case-Mix, Medicare PPS, and Medicaid

Qualifications

• Graduate of an approved RN or LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
• Knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement; OBRA regulations and MDS process.

Responsibilities

• Assists in ensuring documentation meets Federal, State regulations and certification.
• Coordinates the RAI process from MDS through care plan completion.
• Monitors and follows up on RAI process tracking and Medicaid forms in the PointClickCare system.
• Reports trends from audits to the Quality Assurance Committee.
• Ensures timely submission of MDSs to the State with validation record maintenance.
• Follows up with staff to assure compliance to documentation standards.
• Completes patient assessments and data collection per the MDS User’s Manual.
• Determines the Assessment Reference Date reflecting care needs and resources for Medicare/Medicaid payments.
