Job summary

Focused Post Acute Care Partners FPACP is seeking an MDS Coordinator in Orange, TX. You will assist the Director of Nursing to ensure MDS documentation aligns with federal, state and certification guidelines and coordinate RAI, MDS, CAAS and interdisciplinary care plans.

Role requires an LVN license and skilled nursing facility experience. The position emphasizes regulatory compliance, data collection, and collaboration with the care team; benefits and a supportive culture are highlighted.

Qualifications

• Must have a current LVN license.
• Minimum of two years nursing experience in a skilled nursing facility preferred.
• Excellent knowledge of RAI Process, the federal Medicare PPS process, and Medicaid reimbursement as required.
• Thorough understanding of the Quality Indicator process.
• Knowledge of OBRA regulations and Minimum Data Set.
• Knowledge of the care plan process.

Responsibilities

• Assist the Director of Nursing with ensuring that documentation meets federal, state, and certification guidelines.
• Coordinate the RAI process assuring timeliness and completeness of the MDS, CAAS, and Interdisciplinary Care Plans.
• Ensure adherence to regulatory and certification survey processes and correct deficiencies.
• Provide data for audits and report trends to Quality Assurance.
• Ensure timely submission of MDS and care plans with proper signatures and documentation.
