Job summary

Elevance Health is seeking a LTSS Service Coordinator to coordinate services for a defined caseload in specialized programs. You will conduct assessments, document preferences and develop a comprehensive PCSP with input from the member and care team.

The role requires travel to multiple counties in Region B, with a field-based schedule and occasional office attendance for meetings or training. A hybrid/onsite work policy applies, with commuting distance considerations.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Bilingual in English/Spanish strongly preferred.

Responsibilities

• Perform face-to-face program assessments using pre-defined questions and motivational interviewing.
• Coordinate and manage waiver cases (LTSS/IDD, BH or PH needs) as the single point of contact with the care team.
• Document member goals and care plans with the member, caregivers, and providers.
• Identify members who may benefit from alternative service levels or other waiver programs.
• May mentor new staff and participate in process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies as applicable.
• Assist with appeals, grievances and state audits.
