Job summary

Elevance Health is seeking an LTSS Service Coordinator (Bilingual in Spanish/English) to provide coordinated services for a defined caseload in specialized programs. You will conduct assessments, help develop person-centered plans, coordinate care with medical directors, and work with the member's support circle to manage PH/BH/LTSS needs.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments and coordinate LTSS/Waiver needs with the interdisciplinary team.
• Identify members needing alternative levels of service or other waiver programs.
• Document member goals in collaboration with care teams and prepare PCSPs and backup plans.
• Submit utilization/authorization requests and report critical incidents to appropriate parties.
• Assist with appeals, grievances, and state audits and mentor new staff as needed.
