Job summary

Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) to conduct service coordination for a defined caseload in specialized programs. This field-based role travels across multiple counties to assess needs and develop integrated care plans with a person-centered approach.

The position requires a BA/BS and at least 2 years in social work. A hybrid/remote work policy applies, with some office attendance and commuting flexibility as needed.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments using pre-defined questions and motivational interviewing.
• Coordinate and manage a member's waiver and BH/PH needs with the clinical team.
• Manage non-clinical needs of members to ensure cost-effective LTSS utilization.
• Document short and long-term service goals with the member and care team.
• Identify members who could benefit from alternate service levels or other waivers.
• Mentor or preceptor for new staff and participate in process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies.
• Assist with appeals, grievances, and state audits.
