Job summary

Elevance Health is seeking a Field-based LTSS Service Coordinator (Bilingual in Spanish/English) to conduct assessments, coordinate care, and manage a defined caseload in multiple counties across Region B. This role requires travel to client sites and close collaboration with the member’s care team.

The coordinator will document person-centered plans, engage with the member’s circle of support, and ensure alignment with state and contractual requirements while maintaining compliance and quality

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency or equivalent.
• Bilingual in English/Spanish strongly preferred.
• LTSS, case management, social work or hospital discharge planning experience preferred.

Responsibilities

• Perform face-to-face program assessments for identification and case management of LTSS/IDD, BH or PH needs.
• Coordinate members’ cases with clinical teams to provide integrated care.
• Document member goals in care plans in collaboration with the care team.
• Identify members who may benefit from alternative service levels or other waiver programs.
• Support training, process improvement, and utilization and authorization processes.
