Job summary

Elevance Health seeks a Field-based LTSS Service Coordinator to manage a defined caseload, conducting assessments and developing Person Centered Plans. The role involves coordinating with care teams, documenting goals, and ensuring integrated care across PH/BH/LTSS needs.

Travel to client sites is required, with occasional office attendance. The ideal candidate will have a BA/BS and at least 2 years in social services, with Spanish/English bilingual abilities highly preferred.

Qualifications

• The position requires a BA/BS degree and a minimum of 2 years of experience working with a social work agency or similar environment.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments using predefined questions and motivational interviewing techniques for waiver identification and care coordination.
• Coordinate member cases as the single point of contact with clinical teams to address high‑risk health care needs and gaps in care.
• Manage non-clinical needs of members with chronic illnesses to ensure efficient utilization of LTSS.
• Document short- and long-term service goals with the member and care team, including caregivers and physicians.
• Identify members who may benefit from alternative levels of service or other waiver programs.
• Mentor or precept new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with supporting documentation aligned to the care plan.
• Report critical incidents to state and county agencies as required.
• Assist in member grievances, appeals, and state audits as needed.
