Job summary

Qualifications

• BA/BS degree required; minimum 2 years experience with a social work agency.

Responsibilities

• Conduct face-to-face program assessments and coordinate LTSS cases.
• Document member goals and care plans with the care team and providers.
• Identify members who may need alternative services or other waivers.
• Support the member’s care team through coordination with clinical and interdisciplinary teams.
• Assist in appeals, grievances, audits, and utilization management as needed.
