Elevance Health is seeking an LTSS Service Coordinator (Bilingual English/Spanish) to support a defined caseload in specialized programs. The role involves field-based assessments, service coordination, and developing Person Centered Plans in collaboration with the member and care team.

The position requires a BA/BS and at least 2 years of social work experience, with a strong preference for bilingual candidates. Travel to multiple counties is required, with some office attendance per policy.

Qualificações

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Bilingual in English/Spanish strongly preferred.
• LTSS, case management or discharge planning experience preferred.

Responsabilidades

• Conduct face-to-face program assessments and coordinate LTSS/IDD or BH/PH needs.
• Serve as single point of contact, coordinating with clinical teams to develop an integrated care plan.
• Document member goals and support plans in collaboration with the care team.
