Job summary

Elevance Health is seeking a LTSS Service Coordinator who will coordinate a defined caseload in specialized programs. This field-based role involves systematizing the Person Centered Planning process, coordinating with the member’s care team, and ensuring integrated PH/BH/LTSS plans.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Perform face-to-face program assessments for identification and coordination of an individual’s LTSS/IDD, BH or PH needs.
• Identify members who may require higher level care and coordinate with the clinical team as the single point of contact.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective LTSS utilization.
• Document members' short and long-term service goals with their care team and supports.
• Identify members who may benefit from alternative levels of service or other waiver programs.
• Mentor or preceptor role for new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to appropriate state and county agencies.
• Assist with appeals, grievances, and state audits.
