Job summary

Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) to operate primarily in the field, traveling to client sites across multiple counties in Region B. The role supports person-centered planning and integrated care for members with LTSS/BH/PH needs.

The position requires a BA/BS and at least 2 years of social work experience, with strong Spanish/English communication. Hybrid/office attendance policies apply, and compensation includes a comprehensive benefits

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience in a social work setting.
• Bilingual in English/Spanish strongly preferred.
• Experience with LTSS, case management, or discharge planning preferred.

Responsibilities

• Conduct face-to-face program assessments using predefined tools and motivational interviewing.
• Coordinate and manage an individual’s waiver and BH/PH needs with the care team.
• Document short- and long-term goals in the Person Centered Support Plan with the member’s circle of support.
• Identify members who may benefit from alternative services or other waiver programs.
• Support training of new staff and participate in process improvement initiatives.
