Job summary

Elevance Health seeks an LTSS Service Coordinator to conduct service coordination for a defined caseload within specialized programs. The role involves Person Centered Planning, assessments, and development of PCSP and backup plans, interfacing with Medical Directors and interdisciplinary care rounds to support integrated care plans.

Travel across multiple counties is required, with a field-based focus and occasional office attendance.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.
• Bilingual in English/Spanish strongly preferred.

Responsibilities

• Conduct face-to-face program assessments using predefined questions to identify needs and plan care.
• Coordinate member cases with clinical teams to provide care coordination support.
• Document member goals and service planning with the care team and caregivers.
• Identify candidates who could benefit from alternative levels of service or other waiver programs.
• Support in training new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests and document alignment with care plans.
• Report critical incidents to state and county agencies as required.
