Job summary

RHA Health Services, LLC seeks an IDD Qualified Professional to coordinate, monitor, and support person-centered services for individuals with IDD within Forsyth I & Forsyth II facilities. The role focuses on PCP development, admissions, and ensuring occupancy while aligning with regulatory requirements.

The QP collaborates across nursing, direct support, and guardians to drive outcomes, with emphasis on independence and well-being. Requires a Bachelor’s in a related field; Master’s preferred.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Special Education, or a related field (Master's degree preferred).
• Excellent communication, organizational, and documentation skills.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Lead intake and admission processes, pre-admission screenings, and PCP development.
• Coordinate the admission process and ensure timely documentation.
• Develop and implement PCP upon admission with the IDT.
• Monitor PCP implementation and progress toward outcomes.
• Conduct regular site visits to evaluate PCP delivery and quality.
• Communicate with families, guardians, and stakeholders to keep them informed.
• Maintain timely and accurate Medicaid/ICF documentation and participate in training.
