Job summary

RHA Health Services, LLC is seeking a Qualified Professional to join our IDD care team in Forsyth I & Forsyth II. You will coordinate PCPs, manage admissions, and ensure bed occupancy while aligning services with each person's goals and needs.

You will collaborate with nursing, direct support, clinical staff, and guardians to drive person-centered care, regulatory compliance, and quality outcomes across the facility.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field (Master’s degree preferred).
• Experience developing and implementing Person-Centered Plans (PCPs).
• Strong written and verbal communication, organization, and documentation skills.

Responsibilities

• Lead admissions and census management, including pre-admission screenings and PCP development.
• Coordinate IDT meetings and ensure PCP fidelity and progress toward outcomes.
• Monitor and review service delivery for regulatory compliance and quality improvement.
• Maintain communication with families/guardians and advocate for individuals served.
