Job summary

RHA Health Services, LLC is hiring an IDD Qualified Professional ICF to coordinate, monitor, and support person-centered services for individuals with IDD. You will lead admissions, PCP development, and ensure bed occupancy targets are met, collaborating with nursing, DSPs, and guardians.

The role emphasizes independence, well-being, and inclusion, with on-site schedule 9am–5pm and a monthly on-call rotation. Prior IDD experience and knowledge of PCPs are essential.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field.
• 2+ years experience with IDD in NC; 1 year if Master’s degree.
• Experience developing, writing, and implementing PCPs.
• Knowledge of ICF regulations and Medicaid/HCBS rules.
• Strong communication, organizational, and documentation skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• Must be at least 18 years old.

Responsibilities

• Lead intake and admission process, develop PCPs on admission.
• Coordinate IDT meetings and document timelines.
• Monitor PCP fidelity and progress toward outcomes.
• Maintain communication with families and guardians.
• Complete documentation per Medicaid/ICF requirements.
