Job summary

RHA Health Services, LLC is seeking a Qualified Professional to coordinate person-centered services for individuals with IDD and mental health needs in North Carolina. You will lead admissions, PCP development, and active bed occupancy management within an IDT, ensuring services align with each person’s goals and needs.

You will collaborate with nursing, direct support staff, guardians, and clinicians to drive regulatory compliance and quality outcomes, while maintaining strong relationships

Qualifications

• Bachelor’s degree required; Master’s degree preferred.
• Minimum 2 years experience with IDD in NC; 1 year if Master’s degree.
• Experience in developing, writing, and implementing Person-Centered Plans (PCPs).

Responsibilities

• Lead admissions and census management, including pre-admission screenings and PCP development.
• Coordinate IDT meetings and ensure documentation aligns with timelines.
• Monitor PCP fidelity and progress toward personal, clinical, and functional outcomes.
• Oversee site visits to ensure safety, engagement, and community integration.
