Job summary

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Special Education, or related field; Master's preferred.
• At least 2 years of experience in North Carolina (1 year in Tennessee) working with individuals with IDD; 1 year required if Master's degree.
• Experience developing, writing, and implementing Person-Centered Plans.
• Working knowledge of ICF regulations, Medicaid/HCBS rules, and related requirements.
• Excellent communication, organizational, and documentation skills.

Responsibilities

• Lead the intake and admission process, including pre-admission screenings and meet-and-greet visits; determine program compatibility.
• Develop and implement the initial PCP upon admission in collaboration with the IDT.
• Coordinate IDT meetings, document outcomes, and ensure PCP fidelity.
• Monitor PCP delivery and progress toward identified outcomes; maintain regulatory compliance.
• Conduct site visits to evaluate quality, safety, and community integration; manage documentation per Medicaid/ICF rules.
