Job summary

RHA Health Services, LLC is hiring an IDD Qualified Professional ICF to oversee admissions, service coordination, and PCP development. The role ensures timely intake, compliance with regulations, and ongoing monitoring of service delivery in various settings.

The position emphasizes collaboration with families, guardians, and the IDT to align care with person-centered goals, maintain documentation, and support program occupancy and quality improvement initiatives.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field (Master’s degree preferred).
• Excellent communication, organizational, and documentation skills.

Responsibilities

• Lead admissions and census management, including pre-admission screenings and PCP development.
• Coordinate IDT meetings and ensure PCP implementation aligns with assessments and goals.
• Conduct regular site visits to ensure PCP fidelity, safety, and community integration.
• Maintain strong communication with families and guardians to involve them in planning and care decisions.
• Complete timely documentation in accordance with Medicaid, ICF, and company requirements.
