Job summary

RHA Health Services, LLC in Georgia is seeking a Qualified Professional (QP) to coordinate person-centered services for individuals with IDD in an ICF setting. You will lead admissions, develop the initial PCP with the IDT, and actively manage bed occupancy through proactive referrals.

You will partner with nursing, direct care, clinicians, and guardians to ensure goals and support needs are met while maintaining regulatory compliance and high-quality service delivery.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or a related field.
• Master’s degree preferred.
• Experience developing, writing, and implementing Person-Centered Plans.
• Knowledge of ICF regulations and Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.

Responsibilities

• Lead the intake and admission process, including pre-admission screenings and meet-and-greet visits.
• Coordinate all aspects of the admission process to ensure timely occupancy.
• Develop and implement the initial PCP upon admission with the IDT.
• Monitor PCP implementation and progress toward outcomes.
• Maintain communication with families, guardians, and external stakeholders.
• Ensure timely and accurate documentation per Medicaid, ICF, and company requirements.

Education

Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or a related field
