Job summary

RHA Health Services, LLC is hiring a Full-Time Qualified Professional (QP) for an ICF setting in North Carolina. You will coordinate, monitor, and support person-centered services, developing PCPs and leading the referral and admission process to maintain full bed occupancy.

You will collaborate with nursing, direct support, clinical specialists, and guardians, ensuring regulatory compliance and person-centered outcomes for individuals with IDD and mental health needs.

Qualifications

• Bachelor’s degree in a relevant field with preferred Master’s degree.
• Experience developing, writing, and implementing Person-Centered Plans.
• Knowledge of ICF regulations, Medicaid/HCBS rules, and IDD supports.

Responsibilities

• Lead admissions and census management from pre-admission to occupancy.
• Coordinate IDT meetings and translate assessments into PCPs.
• Monitor PCP fidelity and track outcomes across personal, clinical, and functional areas.
• Maintain documentation per Medicaid, ICF, and company requirements.

Education

Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or a related field

Master’s degree (preferred)
