Job summary

RHA Health Services, LLC is seeking an IDD Qualified Professional (QP) for our ICF program. The QP will coordinate person-centered plans, lead the admission and census process, and ensure regulatory compliance while collaborating with the IDT to support independence and inclusion.

The role requires a bachelor’s degree in a related field and experience with PCPs, ICF regulations, and Medicaid/HCBS rules. Travel and driving may be required in the assigned states.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, or related field.
• Master’s degree preferred.
• 2 years of experience in NC, 1 year in TN working with IDD; 1 year if Master’s degree.
• Experience in developing, writing, and implementing Person-Centered Plans (PCPs).
• Knowledge of ICF regulations and Medicaid/HCBS rules.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead admissions and census management, including pre-admission screenings and PCP development at admission.
• Coordinate IDT meetings and ensure timely documentation and PCP implementation.
• Monitor PCP fidelity and outcomes across clinical and personal domains.
• Maintain communication with families, guardians, and external stakeholders for planning and advocacy.
• Perform other duties supporting health, safety, and well-being of individuals served.
