Job summary

RHA Health Services, LLC in North Carolina is hiring a Qualified Professional (QP) for the IDD/ICF setting. The QP coordinates, monitors, and supports person-centered services, develops PCPs, and leads admissions and bed occupancy management within the ICF.

As primary care coordinator, you collaborate with nursing, DSPs, clinical specialists, and guardians to ensure independence, wellbeing, and regulatory compliance. This is a full-time, on-site role with a sign-on bonus.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or a related field.
• At least 2 years of experience in North Carolina with individuals with IDD; Master’s degree reduces to 1 year.
• Experience writing and implementing Person-Centered Plans.
• Knowledge of ICF regulations and Medicaid/HCBS rules.
• Excellent communication and documentation skills.

Responsibilities

• Lead the admissions process, including pre-admission screenings and meet-and-greet visits with individuals and families.
• Coordinate all admission aspects and ensure timely documentation and smooth transitions into the facility.
• Develop and implement the initial Person-Centered Plan (PCP) upon admission in collaboration with the IDT.
• Monitor PCP implementation and occupancy, and support audits and corrective action plans as needed.
