Job summary

ExcelliSearch, Inc. seeks a Hospital Liaison for a full-time, on-site role in New York. You will direct post-acute intake, drive census growth, and leverage hospital relationships across Brooklyn and Queens to expand the facility’s referral footprint.

You will conduct pre-admission evaluations, coordinate with case managers and physicians, and partner with admissions, nursing, and therapy teams to ensure smooth patient transitions. A 2+ year SNF/LTC background is required.

Qualifications

• Required SNF/LTC experience: minimum 2+ years in a liaison/admissions role.
• Territory network: hospital relationships with case management teams in Brooklyn and Queens.
• Education & credentials: Bachelor's degree; active clinical credential a plus.
• Industry & payer knowledge: understanding of discharge workflows, Medicare Part A/Medicaid, and insurance pre-authorizations.
• Core competencies: consultative sales, clinical evaluation, communication, accountability.

Responsibilities

• Territory & network expansion with hospital case managers, discharge planners, social workers, and hospitalists in Brooklyn/Queens.
• Clinical pre-screening: pre-admission evaluations and chart reviews for medical stability.
• Census & referral generation: convert clinical leads into admissions to meet targets.
• Patient & family guidance: explain sub-acute rehab programs and transitions.
• Interdisciplinary collaboration: coordinate with admissions teams, nursing directors, and therapy leaders for hand-offs.
• Market intelligence: monitor local trends and present to hospital teams.
