Job summary

Acloché LLC is seeking a Hospital Liaison to direct post-acute intake and drive census growth across major NYC health systems. This dual clinical sales and admissions leadership role involves leveraging hospital relationships to identify prospective SNF/LTC patients and to guide transfers with case managers, discharge planners, and physicians.

You will perform pre-admission evaluations, expand referral footprints, and collaborate with nursing, therapy, and admissions teams to secure approvals

Qualifications

• Experience in SNF/LTC referral production and admissions.
• Strong knowledge of hospital discharge workflows and payer requirements.
• Proven ability to develop and maintain hospital relationships.

Responsibilities

• Maintain and grow hospital relationships with case managers and discharge planners.
• Perform pre-admission evaluations and chart reviews for referrals.
• Convert clinical leads into admissions to meet census targets.
• Consult patients and families in hospital settings to outline sub-acute programs.
• Coordinate with admissions teams to ensure smooth hand-offs and pre-authorizations.
• Provide market intelligence on local discharge trends and educational outreach.
