Job summary

CareerNerds, LLC is seeking a Hospital Liaison in the New York City area. This full-time on-site role directs post-acute intake, expands census, and leverages hospital relationships to drive referrals across Brooklyn and Queens.

You will work with case managers, discharge planners, social workers, and physicians to streamline pre-admission evaluations and patient transfers, while coordinating with nursing and therapy leadership to ensure seamless hand-offs and pre-authorizations.

Qualifications

• Minimum 2+ years in SNF/LTC in a liaison/admissions role.
• Active relationships with hospital case management teams in Brooklyn and Queens.
• Bachelor’s degree; professional clinical credential (RN/LPN/MSW/LCSW/PT/OT) a plus.

Responsibilities

• Territory & Network Expansion: Maintain relationships with hospital staff across Brooklyn and Queens.
• Clinical Pre-Screening: Perform pre-admission evaluations and chart reviews.
• Census & Referral Generation: Convert leads into admissions to meet census targets.
• Patient & Family Guidance: Explain sub-acute programs and transitions to patients/families.
• Interdisciplinary Collaboration: Coordinate with admissions, nursing, and therapy teams.
• Market Intelligence: Monitor local trends and deliver education to case management teams.
