Job summary

Essenti Talent in New York seeks a Hospital Liaison to lead post-acute intake and grow census across NYC health systems. In a dual clinical sales and admissions leadership role, you will leverage hospital relationships to identify prospective SNF and LTC patients, perform pre-admission evaluations, and streamline transfers.

You will work with case managers, discharge planners, social workers, and physicians to expand referral footprints, secure pre-authorizations, and hand off care plans.

Qualifications

• Minimum 2+ years of SNF/LTC experience in liaison/admissions roles.
• Proven hospital relationships in Brooklyn and Queens.
• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field.
• Clinical credentials (RN/LPN/MSW/LCSW/PT/OT) are a strong plus.
• Knowledge of Medicare Part A, Managed Medicaid, and commercial pre-authorizations.
• Strong consultative sales, clinical evaluation, communication, and accountability.

Responsibilities

• Expand territory and network with hospital case managers and discharge planners.
• Conduct pre-admission evaluations and chart reviews.
• Drive census growth and exceed monthly targets.
• Guide patients and families through sub-acute programs.
• Collaborate with nursing, admissions, and therapy leaders.
• Monitor discharge patterns and present to hospital management.
