Client Growth Resources, Inc. is seeking a Hospital Liaison to direct post-acute intake and drive census growth across Brooklyn and Queens.

This dual clinical sales and admissions leadership role will leverage hospital relationships to identify sub-acute and long-term care patients and act as the primary advisor to hospital staff on pre-admission evaluations and smooth transfers. The role emphasizes collaboration with hospital case managers, discharge planners, and physicians, with targets for

Qualifications

• Required SNF/LTC experience: 2+ years in liaison, admissions, or healthcare business development.
• Territory network with hospital case management teams in Brooklyn and Queens.
• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field preferred; active clinical credential a strong plus.
• Deep understanding of acute hospital discharge workflows, SNF/LTC admission criteria, Medicare Part A, Medicaid, and pre-authorizations.
• Strong consultative sales ability, decisive clinical evaluation, excellent communication, and professional accountability.

Responsabilités

• Territory & Network Expansion: Maintain high-value relationships with hospital staff in Brooklyn and Queens.
• Clinical Pre-Screening: Perform pre-admission evaluations and chart reviews for medical stability and care needs.
• Census & Referral Generation: Convert clinical leads into admissions to meet census targets.
• Patient & Family Guidance: Advise patients and families on sub-acute programs and transitions.
• Interdisciplinary Collaboration: Work with admissions teams, Directors of Nursing, and therapy leaders to coordinate care plans.
• Market Intelligence: Track discharge patterns and present to hospital management.
