Job summary

Job Juncture in New York is seeking an experienced Hospital Liaison to direct post-acute intake and drive census growth across NYC health systems.

In this dual clinical sales and admissions leadership role, you will leverage hospital relationships to identify prospective SNF/LTC patients, perform pre-admission evaluations, and expand the facility's referral footprint.

Qualifications

• Minimum 2+ years in SNF or LTC in a liaison/admissions role.
• Active relationships with hospital case management teams in Brooklyn and Queens.
• Bachelor’s degree in healthcare-related field preferred; clinical credential a strong plus.
• Knowledge of Medicare Part A, Medicaid, and pre-authorizations.

Responsibilities

• Territory & Network Expansion with hospital stakeholders in Brooklyn and Queens.
• Clinical Pre-Screening to assess medical stability and care needs.
• Census & Referral Generation to meet monthly targets.
• Patient & Family Guidance in hospital settings for sub-acute programs.
• Interdisciplinary Collaboration with admissions, nursing, and therapy leaders.
• Market Intelligence through local healthcare trends and presentations.
