Professional Recruiting Consultants, Inc. is seeking a Hospital Liaison for a premier skilled nursing and sub-acute organization in the New York City area.

This full-time, on-site role directs post-acute intake and drives census growth by leveraging hospital relationships across Brooklyn and Queens. You will perform pre-admission evaluations, streamline patient transfers, and collaborate with admissions, nursing, and therapy leaders to ensure seamless transitions and secure referrals.

Formación

• Minimum 2+ years direct SNF/LTC liaison or admissions experience in a healthcare setting.
• Established hospital relationships with case management teams in Brooklyn and Queens.
• Bachelor’s degree in a related field; active clinical credential is a strong plus.
• Deep knowledge of hospital discharge workflows, SNF/LTC admission criteria, and pre-authorizations.
• Strong consultative sales ability and excellent communication.

Responsabilidades

• Territory & network expansion across Brooklyn and Queens with hospital staff.
• Clinical pre-screening and chart review to determine medical stability and care needs.
• Convert clinical leads into admissions to achieve census targets.
• Guide patients and families in hospitals about sub-acute rehabilitation options.
• Collaborate with admissions teams, nursing leadership, and therapy leaders for hand-offs and pre-authorizations.
• Monitor local healthcare trends and share market insights with leadership.
