Job summary

MDS Staffing is seeking a Hospital Liaison for the New York area. This on-site, full-time role focuses on post-acute intake and census growth by leveraging hospital relationships across Brooklyn and Queens.

You will lead pre-admission evaluations, facilitate smooth patient transfers, and expand the facility’s referral footprint. The position requires 2+ years in SNF/LTC settings, a bachelor’s degree in a healthcare-related field, and clinical credentials are a strong plus.

Qualifications

• Minimum 2+ years in SNF/LTC as a liaison, admissions, or business development role.
• Active hospital relationships with case management teams in Brooklyn and Queens.
• Bachelor’s degree; clinical credential (RN, LPN, MSW, LCSW, PT or OT) is a strong plus.
• Understanding of Medicare Part A, Managed Medicaid, and payer pre-authorizations.

Responsibilities

• Maintain high-value hospital relationships across NYC health systems to drive referrals.
• Perform pre-admission evaluations and chart reviews to assess clinical fit.
• Convert leads into admissions to meet census and occupancy targets.
• Guide patients and families through sub-acute programs and transitions.
• Collaborate with admissions, nursing, and therapy leaders to coordinate care plans and pre-authorizations.
• Monitor discharge trends and present education to hospital case management teams.
