Job summary

VitalCaring Group is seeking a Care Transition Coordinator – Hospice to connect patients, families, and healthcare partners with hospice services. This field-based role emphasizes relationship-building, care coordination, and growth through referrals.

You will serve as the primary liaison between referral sources and clinical teams, ensuring timely transitions of care and strong partnerships across hospitals, SNFs, and physician offices.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience).
• Minimum of two years healthcare or related sales experience.
• Strong communication, relationship-building, and influencing skills.
• Proficiency with Microsoft Office and CRM systems.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute structured, effective sales calls including pre-call planning, relationship building, needs discovery, and closing.
• Build and maintain strong relationships with referral partners through consistent communication and service excellence.
• Develop and execute strategies to grow market share and drive referral volume.
• Track, manage, and document sales activity across referral sources using CRM tools.
• Achieve established referral and admission goals.
