Job summary

VitalCaring Group is seeking a Care Transition Coordinator to connect patients, families, and healthcare partners with hospice services. This field-based role emphasizes relationship-driven care transitions and proactive coordination across teams to improve patient access and outcomes.

You will manage referrals, coordinate admissions, and partner with hospitals and physicians to grow the network while maintaining compliance and high-quality care. Travel within the assigned territory is expected.

Qualifications

• Bachelor’s degree preferred (or equivalent healthcare sales experience).
• Minimum of two years in healthcare or related sales.
• Strong communication and relationship-building abilities.
• Willingness to travel and work in a field-based environment.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams.
• Execute structured sales calls including planning, discovery, and closing.
• Build and maintain relationships with referral partners for growth.
• Develop strategies to increase market share and referral volume.
• Track activity across referral sources using CRM tools and document outcomes.
• Meet referral and admission goals and support coordinated care.
