VitalCaring Group seeks a Care Transition Coordinator to connect patients, families, and partners with hospice services. This field-based role blends healthcare sales, care coordination, and partnerships to drive patient access and organizational growth.

The coordinator acts as the primary liaison between referrals and internal teams, ensuring seamless transitions and timely admissions while maintaining strong relationships with hospitals and providers.

Qualifications

• Bachelor’s degree preferred (or equivalent experience) in a related field.
• 2+ years in healthcare or related sales experience.
• Strong communication and relationship-building skills.
• Ability to manage multiple priorities in a field-based role.
• Proficiency with Microsoft Office and CRM systems.

Responsibilities

• Liaise between referral sources, patients, caregivers, and clinical teams.
• Execute sales calls: planning, relationship-building, discovery, closing.
• Maintain relationships with referral partners and ensure service excellence.
• Grow market share and drive referral volume strategies.
• Track and document sales activity across referral sources using CRM.
• Meet or exceed referral and admission goals.
• Build partnerships with hospitals, physicians, case managers, and stakeholders.
• Collaborate with clinical/operational teams for timely admissions.
• Represent VitalCaring professionally across healthcare settings.
• Engage in community outreach and promotional events.
