VitalCaring Group is seeking a Care Transition Coordinator – Hospice to join our field-based team. You will connect patients, families, and healthcare partners to ensure seamless transitions into home health or hospice services, serving as the primary liaison between referral sources and clinical teams.

This role blends healthcare sales, care coordination, and partnership development to support patient outcomes and organizational growth.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience).
• Minimum of two (2) years of healthcare or related sales experience
• Strong communication, relationship-building, and influencing skills
• Ability to manage multiple priorities in a fast-paced, field-based environment
• Proficiency with Microsoft Office and CRM systems
• Demonstrated ability to work independently and drive results
• Valid driver’s license and reliable transportation

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions
• Execute structured, effective sales calls including pre-call planning, relationship building, needs discovery, and closing
• Build and maintain strong relationships with referral partners through consistent communication and service excellence
• Develop and execute strategies to grow market share and drive referral volume
• Track, manage, and document sales activity across referral sources using CRM tools
• Achieve established referral and admission goals
