Job summary

Enhabit Home Health & Hospice is seeking qualified healthcare professionals to review and approve patient information at start of care, re-certifications, and evaluations, partnering with branch leadership to educate LPs and ensure regulatory documentation standards.

Ideal candidates hold a professional nursing or physical therapy license, have at least one year of experience in Medicare home health or hospice, and possess solid technology skills including Microsoft Office; in-office attendance

Qualifications

• Must be a graduate of an approved school of professional nursing or physical therapy.
• Must maintain a valid clinical license per state requirements.
• Minimum of one year of experience.
• CPR certification and a valid state driver license are required.
• Experience in a Medicare home health or hospice setting preferred.

Responsibilities

• Review and approve patient information submitted by the Licensed Professional (LP) at start of care, re-certification, resumption of care, or evaluation visit.
• Partner with branch leadership to train LPs and ensure documentation meets regulatory standards.
• Provide in-depth training to LPs when needed and promote quality patient care and strong relationships with referrals, patients, and caregivers.
