Job summary

Enhabit Home Health & Hospice is seeking qualified professionals to review and approve patient information at start of care and recertification, ensuring regulatory standards are met. The role includes coaching LPs, providing in-depth training when needed, and maintaining strong relationships with referral sources, patients, caregivers and staff across the branch network.

Qualifications

• Must be a graduate of an approved school of professional nursing or physical therapy.
• Must maintain a valid license per state requirements.
• Minimum one year of experience in Medicare home health or hospice required.
• CPR certification and valid state driver's license required.

Responsibilities

• Review and approve patient information submitted by the LP during start of care, recertification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs to ensure documentation meets regulatory standards.
• Meet one-on-one with LPs when necessary to provide in-depth training.
• Promote quality patient care and maintain strong relationships with referral sources, patients, caregivers and employees.
