Job summary

Enhabit Home Health & Hospice is seeking a licensed professional (nurse or physical therapist) to review and approve patient information at the start of care, recertifications, resumption of care, or evaluation visits. You will educate LPs and ensure documentation meets regulatory standards.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license required by state rules.
• Minimum one year of experience.
• Experience in Medicare home health or hospice preferred.
• CPR certification required.
• Valid state driver’s license and automobile liability insurance required.
• Basic technology skills; proficiency with Microsoft Word and Excel.
• Experience with Homecare Homebase (HCHB) preferred.
• In-office attendance required for most positions; remote work not allowed.

Responsibilities

• Responsible for the review and approval of all patient information submitted by the Licensed Professional (LP) during a start of care, re-certification, resumption of care, or evaluation visit.
• Partners with branch leadership to provide ongoing education and training to all branch LPs to ensure staff understand documentation requirements in an effort to meet regulatory standards and to keep staff abreast of changes.
• When necessary, meet one-on-one with individual LPs to provide in-depth training.
• Promotes quality patient care; ensures stable relationships with referral sources, patients, caregivers and employees
