Optimal Care in Kalamazoo, MI is seeking a Physician Services clinician to deliver advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings, with autonomy to focus on thoughtful, patient-centered care.

You will lead medical care coordination, supervise Nurse Practitioners and Physician Assistants, mentor colleagues, and help reduce hospitalizations while improving quality of life for patients and families.

Qualifications

• MD/DO required and licensure in good standing
• Current unrestricted State license to practice medicine
• Board certification in family practice, internal medicine, geriatric or palliative care
• Prior supervisor/management experience
• Reliable transportation, valid driver’s license, and automobile insurance

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings.
• Develop individualized treatment plans and coordinate care with patients, families, and interdisciplinary teams.
• Provide medical leadership and mentorship to Nurse Practitioners and Physician Assistants to ensure high-quality, coordinated care.
