Optimal Care in Michigan seeks a Nurse Practitioner or Physician Assistant to provide primary and consultative care to geriatric and medically complex patients, often in homes or care settings, with autonomy to focus on high-quality, patient-centered care that improves comfort and function.

You will conduct medical evaluations, develop treatment plans, prescribe medications, and collaborate with physicians and caregivers in a collaborative practice to avoid unnecessary hospitalizations.

Qualifications

• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage.
• Minimum 2 years clinical experience in post-acute or geriatric care (preferred).
• Adult, Family, or Gerontology certification (preferred).
• Experience with geriatric patients.

Responsabilités

• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Prescribe and monitor medications; coordinate with physicians, caregivers, and interdisciplinary team.
• Serve as a key clinical decision-maker to improve patient outcomes and reduce hospitalizations.
• Explain diagnoses and care plans to patients and families, ensuring understanding and adherence.
• Travel daily within service area to provide home-based or in-facility care.
