Job summary

Optimal Care is seeking a Physician to provide advanced primary and specialty care to geriatric and medically complex patients, often in home or care settings, with autonomy to focus on patient-centered outcomes.

You will conduct comprehensive medical evaluations, diagnose and manage conditions, develop individualized plans, and coordinate care with patients, families, and interdisciplinary teams, while mentoring Nurse Practitioners and Physician Assistants.

Qualifications

• MD or DO with unrestricted state license.
• Board certification in family medicine, internal medicine, geriatric, or palliative care.
• Prior supervisory/management experience and valid driver’s license.

Responsibilities

• Perform comprehensive medical evaluations and diagnose acute and chronic conditions.
• Develop individualized treatment plans and coordinate care with patients, families, and interdisciplinary teams.
• Provide medical leadership and mentorship for NPs and PAs.
• Improve patient outcomes and reduce unnecessary hospitalizations.
