Job summary

Optimal Care is seeking a Physician Services clinician to deliver advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings. You’ll diagnose, evaluate, and manage conditions while coordinating with families and interdisciplinary teams.

In this role you’ll lead NPs and PAs, mentor colleagues, and help improve patient outcomes. Expect meaningful patient interactions within a supportive, clinician-owned organization.

Qualifications

• Interpersonal skills, supervisory and management abilities.
• Organizational development and project management experience.
• Working knowledge of regulations and reimbursement in home health care.

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in home or care settings.
• Conduct comprehensive medical evaluations and manage acute and chronic conditions.
• Develop individualized treatment plans and coordinate care with patients, families, and interdisciplinary teams.
• Provide medical leadership and collaborative oversight for NPs and PAs; mentor clinicians.
• Travel within service area to deliver care to patients in homes, assisted living facilities, or nursing homes.

Education

Doctor of Medicine or Osteopathy

Current unrestricted State license to practice medicine

Board certification in family practice, internal medicine, geriatric or palliative care

Prior supervisor/management experience

Reliable transportation, valid driver’s license, and valid automobile insurance
