Job summary

Optimal Care in Grand Rapids, MI, seeks a Physician to deliver advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings, with autonomy to focus on patient-centered care that improves quality of life.

You will conduct comprehensive evaluations, diagnose and manage conditions, develop treatment plans, and coordinate care with patients, families, and interdisciplinary teams, while mentoring Nurse Practitioners and Physician Assistants.

Qualifications

• MD or DO required for medical practice.
• Current unrestricted State license to practice medicine.
• Board certification in family practice, internal medicine, geriatric or palliative care.
• Reliable transportation, valid driver’s license, and automobile insurance.

Responsibilities

• Conduct comprehensive medical evaluations and manage acute/chronic conditions.
• Develop individualized treatment plans and coordinate care with patients, families, and interdisciplinary teams.
• Provide medical leadership and mentoring for NPs and PAs to ensure high-quality, coordinated care.
